In preparation for the implementation of requiring a National ID for all government-related transactions, the Club has coordinated with the Philippine Statistics Authority (PSA) to arrange a one-day National ID registration at the Main Lounge of the Club on 18 June 2022.
Here are the steps to safely and conveniently secure your National ID:
1. Pre-register via this link.
2. Wait for your appointment date via email and SMS with a link to the PhilSys Registration Form 1A.
3. Print and manually fill in the form but do not affix your signature yet.
4. Bring the form to MPC on the date of your registration and sign it in the presence of a PSA representative.
5. During registration at the Club, your details and supporting documents will be validated and your biometric information taken via fingerprint and iris scans.
*Please note that only Filipino citizens aged five years old and above are eligible to register.
As the national ID is the valid identity of an individual, it should mirror the filter-free faces of the holder. Please be reminded of the following during your biometrics information process:
• Applicants should wear presentable outfits. Sleeveless clothes and heavy makeup are prohibited.
• Eyeglasses or contact lenses should be removed during iris scanning while earrings, necklaces, and other types of facial piercing must also be taken off.
